Background: Frozen shoulder is a condition of uncertain etiology characterized by pain, restriction of shoulder motion and considerable morbidity that occurs in the absence of intrinsic shoulder pathology. It is a self-limiting cascade with extremely slow recovery of shoulder function. However, 10-15% of the patients develop refractory frozen shoulder that does not respond to conservative management. Aim of our study is to correlate pre-treatment MRI findings with treatment outcome. Hypothesis: Certain pre-treatment MRI findings can be correlated with treatment outcome and maybe associated with poorer outcome of conservative treatment. If proven, MRI can be recommended as an essential screening modality in frozen shoulder. Methods: Non-contrast pre-treatment MRI findings of 65 patients diagnosed with primary idiopathic frozen shoulder were studied for correlation with the outcome of non-operative treatment (Physiotherapy + Intra-articular injection). Outcome was assessed through patient evaluation of shoulder motion, pain and self-assessment questionnaire. Results: Axillary recess joint capsule edema in humeral portion shows negative correlation with improvement of external rotation, abduction and VAS (activity) pain scale. Anterior extracapsular edema shows negative correlation with forward flexion, external rotation and positive correlation with oxford shoulder score. Thickness of coracohumeral ligament shows negative correlation with external rotation. Axillary recess capsular thickness-glenoid portion shows negative correlation with difference of abduction, forward flexion and positive correlation with oxford shoulder score. Height of axillary recess shows positive correlation with abduction, internal rotation and negative correlation with VAS (activity). Discussion: Certain MRI findings can be correlated with the outcome of conservative treatment in patients with primary idiopathic frozen shoulder. Therefore, we can predict the course of management by assessing pretreatment MRI scan. We have noted a few MRI findings also correlate with first evaluation. Conclusion: MRI findings correlating with treatment outcome are joint capsule edema – humeral portion, extracapsular edema – anterior, thickness of glenoid capsule in axillary recess and height of the recess.
